Current Procedural Terminology Code: 99215-UA. This has more detailed history of the condition and involves more medical decision process than CPT 99204. For instant a person who had been diagnosed elsewhere with discharge record but not sufficient for the prescriber to make accurate and medically appropriate clinical decision by moderate examination, investigation of condition, diagnosis or history involved, took longer time, and subject to the prescriber’s discretion. This service will be performed by a Nurse Practitioner.
