LOW INCOME PURCHASE

Sliding Scale

Any individual without insurance who can benefit from our service shall be admitted if the individual agrees to pay for the services using our sliding payment scale. The Project Manager or the Officer-in-Charge shall determine the amount to be paid by the individual using the individual’s income level; in comparative to statistic and guidelines provided by either the Department of Human Resources, Social Security Administration, State Department of Finance, Internal Revenue Service or Bureau of Census. Individual at the lowest poverty level shall pay $1.00 per minute of individual, group, and family therapy and rehabilitation counseling service provided and $5 per minute for all Physician services. There shall be 25% increase for each increase in income level. Those whose household income is above the poverty treshold but less than the next two income level are eligible for this discounted services e. g. if your household is four and your income is greater than the poverty threshold for four member family size but less than the poverty threshold for six member family size.
