Psychiatric Rehabilitation Program
Psychiatric Rehabilitation Services includes (COMAR 10.21.17, 10.21.21.07, 10.21.29.07) but not limited to:
A.Rehabilitation and recovery activities directed towards individualís recovery and the improvement or restoration of skills.
B.Self-care skills; including personal hygiene, grooming, nutrition, dietary planning, food preparation, and self administration of medication.
C.Social skills, including community intergration activities, developing natural supports, developing linkages with and supporting the individualís participation in community activities.
D.Independent living skills, including skills necessary for housing stability, maintenance of minorís living arrangment, community awareness, mobility and transportation skills, money management, accessing available entitlements and resources, activities that support the minorís cultural interest, conflict resolution, anger management, interactive skills with peers and authority figures, maintaining personal living space, maintaining age-appropriate boundaries, maintaining personal safety in a social environment, time management, contructive use of structured and unstructured time, supporting the individual to obtain and retain employment and wellness self-management.
E.Activities that support the individualís cultural interests.
F.Medication services that includes administration of medication under Health Occupation Article of annotated code of Maryland and delegation of a licensed practical nurse or a registered nurse according to provisions of COMAR 10.27.11.
G.Monitoring of somatic and pychiatric medications, including support for the individualís self-administration of prescribed medications, reading the medication label to ensure that each container of medication is cleary labeled with individualís name, the contents, directions for use and expiration date, monitor compliance with instructions appearing on the medication label to the extent possible, ensure that each individual has secure, appropriate and accessible space in which to store medications, observe and document any apparent reactions to medication and either verbally or in writing and in a timely fashion, communicate to the prescribing authority any problems that possibly may be related to the medications and reinforce, with the individual, education on the role and effects of medication in symptom management.
H.Health promotion and training activities such as proper nutrition and exercise.
I.Promotion of individual wellness, self management and recovery that includes education and information regarding mental illness and recovery, support the individual to identify effective strategies to assist the individual to manage the individualís wellness, including at a minimum, the individualís identification of potential challenging symptoms, warning signs of relapse, helpful interventions and other people that individual identifies to support the individualís recovery.
J.Documentation of the individualís wellness self-management strategies which may be incorpoarated in the individualís IRP, Residential Rehabilitation Program managed intervention plan, as outlined in COMAR 10.21.21.08, mental health advanced directive or any combination of these items.
K.Review of the strategies at the time of the IRP review which may be revised or revoked at any time at the request of the individual.
L.Provide on-Call and emergency response 24 hours per day, 7 days per week by telephone, to an individual who has been admited to the PRP and assisting the individual to access, as appropriate, the OMHC, mobile crisis, residential crisis services, hospitals, and other service providers that are designated to provide crisis and emergency care and treatment.
M.Proved linkages that includes making reasonable efforts to maintain communication with the individual and the individualís inpatient treatment team and participating in aftercare or release planning from that setting to the degree permited by the setting.
N.Provide services according to COMAR 10.21.21.09 regarding housing development needs by colaborating with the CSA to access affordable housing in the community, maintaining information about available regular housing that is intergrated thoughout the community, not conditioned upon an individualís participation in mental health treatment or support services and within the price range that is affordable to individuals enrolled in the program.
O.CSC staff in conjunction with the CSA will attempt to develop financial resources to assist individuals in our program temporarily in such circumstances as an individualís hospitalization or loss of job benefits or housemates, establish priorities by which housing financial resources are allocated to individuals.
P.Direct services to individuals that helps them to access and sustain housing, including assistance in making choices about where and with whom to live by developing or restoring the basic social and living skills needed, link, when interested, with other individuals with similar interests and housing requirements, evaluate financial resources, available natural supports, and needed and desired community services and secure available private and public housing financial assistance, develop or restore appropriate basic living skills and supports to keep housing, for example responsibilities for upkeep of the property and personal belongings, development of community rsources and natural supports, housemate conflict resolution, landlord-tenant negotiations regarding such issues as rent, needed repairs, complaints and providing support as determined by the individualís need.
